  • 20% or more discount on all supplements, labs, vitamin injections, and IV infusions.

  • Free sports physicals for members or children of members.

Primary Care Membership Terms

  • The first month of care is $240 for all new patients, with subsequent months being the rate listed in the table.

  • A one-time $150.00 initial registration fee includes: account set-up, patient intake, medical record acquisition with extensive review of history.

    • Families registering 4+ members have a max registration fee of $500.

    • Employer groups registering 5+ employees have a max registration fee of $700.

  • The first 3 months of membership fees + registration fee are due at your first appointment.

  • Memberships are billed monthly after the first 3 months.

  • A 10% discount is applied to all memberships paid annually.

  • A 3-month membership commitment is required. This allows time to complete a comprehensive intake process, medical history review, and multiple in-person interviews/exams during the first few months of care.

  • After the initial 3-month commitment, you may cancel your membership at any time and for any reason by providing 30 days’ advance notice.

General Payment Terms

  • Payment is due at time of service.

  • HSA or FSA funds may be used to pay for membership fees, one-time appointments, procedures, labs, supplements, and medical equipment.

  • Payment methods accepted: cash, check, or credit card.

  • All patients are provided a “superbill” upon request to submit to their health insurance plan for service reimbursement. *Please note - Gannett Health does not guarantee reimbursement from any private insurance carriers, Medicare or Medicaid.
