Primary Care Membership Terms
A one-time $150.00 registration fee includes: account set-up, patient in-take, medical record acquisition with extensive review of history.
Families registering 4+ members have a max registration fee of $400.
Employer groups registering 5+ employees have a max registration fee of $500.
The first 3 months of membership fees + registration fee are due at time of first appointment.
Memberships are billed monthly after the first 3 months.
A 10% discount is applied to all memberships paid annually.
A 3-month membership commitment is required. This allows time to complete: a comprehensive intake process; medical history review; and multiple in-person interviews/exams in the first few months of care.
After the initial 3-month commitment, you may cancel your primary care membership at any time and for any reason by providing 30 days’ advance notice.

Lab Pricing
Members receive a 20% or more discount on all labs. Listed prices do not reflect that discount.
General Payment Terms
Payment is due at time of service.
FSA funds may be used to pay for appointments or membership fees.
HSA or FSA funds may be used to pay for lab studies, supplements, and medical equipment.
Payment methods include cash, check, or credit card.
All patients are provided a “superbill” to submit to their health insurance plan for service reimbursement. We are “in-network” with BCBS WY and United.
